I will discuss “Lessons Learned” from the Better Health Through Housing program, a partnership between the Center for Housing and Health and UI Health that identifies and transitions the chronically homeless from our Emergency Room into permanent supportive housing. I will describe chronic homelessness as a dangerous health condition, the invisibility of the homeless in healthcare, their exorbitant costs & utilization, and the need for the alignment and coordination of healthcare, human services and the criminal justice system. I will also define what “collective impact” is and how it can create more capacity to house the homeless.
