Where are the Telemedicine startups concentrated? which countries, and particularly where in the US?

Australia, is in the beggining of a new wave of government and private investment in Telemedicine that can siginificantly enhance the quality of life of many isolated communities, as well as as a test bed for larger scale applications in the developing world, where impact would most profund.
