when choosing someone to speak for you when you cannot, choose someone who is not a relative to avoid potential conflicts of interest.

If you choose a spouse, you may want to die and be able to refuse life-sustaining treatments, but your spouse may not be ready to let you go, yet has committed to speaking "as if in your shoes." This may be a difficult conflict that you wish to avoid.
